20世纪以来,随着科学技术、制药与研发的不断发展,新药品种和用药方法日益复杂。医师更多地专注于疾病诊断和治疗,较难全面掌握日益剧增的药物知识,使得医疗中不合理用药等情况不断发生,药品不良反应及不合理用药导致的患者住院率和死亡人数也呈不断上升趋势。20世纪80年代,Hepler和Strand两位教授在从事药物经济学等方面的研究时发现,美国医疗费用上涨的原因来自患者发病率、住院率的升高,而发病率和住院率持续升高在一定程度上正是不合理用药导致的。事实上,让具备相关知识的药师参与临床实践就可以避免大多数不合理用药问题。从那时起,Hepler和Strand教授更加认同“药物本身没有剂量,只有患者使用药物时,才有了剂量”。为了满足临床和患者的安全用药需求,药师的工作重心必须从药品供应和调剂转向临床,为患者提供用药指导和监护。这种想法和举措,就是药学监护(Pharmaceutical Care)理念的雏形。
1990年,Hepler和Strand教授在《美国医院药学杂志》上发表了里程碑式的文章——《药学监护中的机会和责任》,他们定义了药学监护的概念,明确提出了21世纪药师的使命是药学监护,呼吁广大药师尽快从以药品调剂为重心的传统角色转型到以患者为中心的监护角色上来,承担起患者疾病治疗结果的责任,终帮助患者提升生活质量。这一理念得到了全世界药师的一致认可。1999年,这两位教授获得了美国药学界荣誉——雷明顿奖章,以表彰他们对药学事业所作出的巨大贡献。
