不管是卧位、坐位或站立位,治疗师应教会患者双手握住前伸,在上肢上举后要保证肩胛骨前伸。双侧肘关节伸直、双手握住后,患者可将其上肢举过头,这个活动可反复进行。康复小组所有成员及其家属应对其进行鼓励。即使是在进行输液时,也可小心进行肩关节正常范围内的运动。教会患者正确进行上肢运动非常重要,否则可损伤肩关节,导致肩痛,使患者不愿意再活动。这项活动也可在治疗过程中进行。
在运动或转移时应注意保护患侧肩关节及手,双手放在前面正中位置,双手握住后可刺激感觉系统,增加对患侧的感知能力。双手前伸可防止肩胛骨甚至整个上肢的后缩,使患者运动变得容易一些。在行走时,双手前伸还可防止上肢联合反应。
由于患者用健手握住患手,在其运动时不能推拉患手,患者其他部位运动更加正常,躯干运动对称,负重能力提高。双手握住还可预防患手挛缩,并可做一些练习来防止肌肉痉挛,拉长痉挛肌肉。在白天患者坐立时,可以双腿交叉,双手握住后放在膝关节上,这样可以保证正常姿势。
