2017年,复旦大学附属华东医院与静安区卫生计生委签署合作协议,正式结成"华东医院--静安区康复医疗联合体",以"注重预防、治疗、康复三结合"为指引,以"建体系、扩队伍、立机制、增能力"为目标,构建符合疾病发展规律的早期、系统、专业、连续的康复医疗服务模式,提升区域康复医疗服务能力和水平。
上海市第三康复医院作为"华东医院--静安区康复医疗联合体"的重要组成部分,2014年转型为康复专科医院以来,通过积极探索康复发展的路径,大力发展特色康复,与华东医院康复医学科合作,参与撰写国家《ICF活动和参与评价量表应用技术指南》,主持撰写《医联体脑卒中康复防治一体化管理指南》,并在静安区北部医联体中充分发挥康复医疗牵头单位的作用,深入北部医联体各成员单位开展康复治疗、康复护理等服务,在区域内多次举办康复培训班,组织康复治疗、护理技术竞赛活动,并在国内率先倡导"脑卒中闭环管理模式",为三级康复医疗服务体系建设作了有益的探索,也对区域康复治疗与护理技术的普及、提升做出了很大贡献,静安区"发挥区域康复优势,全面落实康复分级诊疗"成功入选2019年"上海医改十大创新举措"。在此基础上,上海市第三康复医院以脑卒中的分级康复为重点,根据医联体的工作特点编写此书,以期医联体内各级医疗机构脑卒中康复治疗达到规范化,同质化,使脑卒中病人能在医联体的任何一家医疗机构中根据病情需要都能得到同等水平的康复治疗。
