  2.体格检查PAH的体征多与右心功能不全有关,常见有发绀;颈静脉充盈或怒张;下肢水肿;右心室充盈压升高,可出现颈静脉巨大“a”波;右心室肥厚可导致胸骨左缘膨隆,剑突下出现抬举性搏动;肺动脉瓣区第二心音(P2)亢进;由于肺动脉瓣开放突然受阻出现的收缩早期喷射性喀喇音;肺动脉瓣关闭不全的高调舒张早期杂音;血液反流通过三尖瓣引起的收缩期杂音;第三心音(S3)出现代表右心室舒张充盈压增高及右心功能不全,约38%的患者可闻及右心室第四心音(S4)奔马律等。发绀是PAH的晚期表现,通常是因为伴随循环血管收缩的心排血量明显减低,肺一通气灌注比例失调。扩张的肺动脉压迫可引起喉返神经麻痹,较少见(Ortner综合征)。
  颈静脉检查还可以帮助判断右心房压力:患者采取45°半卧位,量取颈静脉搏动点到胸骨柄之间的垂直距离(单位为cm),加上5cm(代表右心房到胸骨柄的距离),数值即为估测的右心房压力值(单位为cmH2O)。右心房压力是判断患者预后的重要指标。
  有些与PAH相关疾病的特殊体征往往可提示诊断:①如果上下肢均存在杵状指(趾)往往提示存在艾森门格综合征的可能性大;差异性发绀往往是诊断动脉导管未闭合并艾森门格综合征的重要线索。②反复自发性鼻出血、特异性体表皮肤毛细血管扩张往往提示遗传性出血性毛细血管扩张症。③反复出现皮疹、面部红斑、黏膜溃疡、小关节肿胀畸形、雷诺症、脱发等提示结缔组织病。④吸气相湿哕音提示肺间质疾病。⑤如果患者出现蜘蛛痣、肝掌、睾丸萎缩提示肝脏疾病。
  3.危险因素
  (1)既往史:先天性心脏病、结缔组织病、HIV感染史、减肥药物治疗史、肝病及贫血等都是提示PAH病因分类的重要线索,需要采集患者所有的既往史,这些可帮助明确诊断分类,也可帮助发现新的危险因素。
  (2)个人史:需要注意患者有无危险因素接触史,如印刷厂、制鞋厂和加油站工人接触油类物品、HIV感染、同性恋、吸毒及染发剂等特殊接触史。
  (3)婚育史:需要注意女性患者有无习惯性流产史,男性患者的母亲、姐妹等亲属有无习惯性流产等病史。
