本书是一本详细介绍幽门螺杆菌相关知识的医学科普书籍。国内外的研究表明,幽门螺杆菌是慢性活动性胃炎、消化性溃疡、胃恶性肿瘤等疾病的主要致病因子,已被世界卫生组织列为I类致癌因子,而且幽门螺杆菌可能还与某些胃肠外疾病(如动脉粥样硬化相关心血管病、血液系统疾病、皮肤病等)相关,我国幽门螺杆菌的感染率约为40%-80%,幽门螺杆菌已成为全球包括我国在内的感染率的慢性感染病之一。

保志军,医学博士,消化科、老年科主任医师,教授,博士研究生导师。现为复旦大学附属华东医院副院长,上海市老年医学临床重点实验室主任,入选"上海市卫生系统优秀学科带头人"培养计划。
兼任中国老年医学研究机构联盟副主任委员,中国医师协会老年医学分会常委,中国老年学学会老年医学委员会常委,中国老年学学会抗衰老科学委员会常委,中国老年医学学会消化分会委员,上海市老年学学会副会长,上海老年学学会老年医疗保健专业委员会常务副主任委员,上海市静安区科协副主席,复旦大学中西医结合研究院老年病研究所名誉所长。

30多年前,当澳大利亚皇家佩斯医院的两位医生巴里﹒马歇尔(内科住院医生)和罗宾﹒沃伦(病理科医生)从胃炎患者的胃窦黏膜中分离并培养出一种螺旋形、微需氧的微生物时,或许未曾预料,这个帮助他们荣膺2005年诺贝尔生理学与医学奖的细菌,足以颠覆人类对一些传统疾病的认识。
当时人们的传统观点认为:无酸便无溃疡,胃酸有杀菌作用,胃内不适合细菌生长。巴里﹒马歇尔和罗宾﹒沃伦于1983年2月将一篇题目为"Spiral bacteria in gastritis and associated disease(在胃炎和相关疾病中的螺旋菌)"的论文摘要投送至澳大利亚胃肠病学年会,随后不出所料,他们的摘要遭到大会秘书处的拒绝。多年后,成名的巴里﹒马歇尔每次在做相关演讲报告时均会风趣地自嘲说:"我当时是一个名不见经传的小人物(内科轮转医生),这样的发现应该大人物报道才有人相信。"

胃上接食管,下通小肠,口、咽、食管和小肠等部位的细菌均可进入胃内。由于胃腔内酸度很高,在这种环境下,多数细菌均可被胃酸杀灭。目前研究已经确认,健康人胃内有100多个亚类的细菌,主要包括厚壁菌门、变形菌门、拟杆菌门和放线菌门等,幽门螺杆菌(网上参考图)属于变形菌门。相比之下,人们对于幽门螺杆菌以外的其他胃内细菌的了解相当有限。虽然胃酸不足以杀灭胃内所有细菌,但胃酸可以限制胃内微生物的过度生长,减少进入小肠的微生物数量并降低致病菌感染风险。
