伴随我国工业化、城镇化的改变和人口老龄化的不断加剧,居民生活方式、膳食结构、生态环境、食品安全等情况对健康的影响越来越凸显,慢病发病、患病、死亡率呈现快速上升趋势,慢病疾病负担也日益沉重。慢病已成为我国居民的重要死因,并且严重影响其生活质量。未来,随着人口老龄化压力将进一步加重我国慢病负担。数据显示,目前,慢性病的防治占我国医疗费用的80%,死亡人口中有86%死于慢性病。康管理的过程是一个行为改变的过程,《承德市医疗康养产业建设研究》,课题组结合承德市市民需求及京津冀康养产业转移的需求构建医养结合养老模型,此方案通过健康管理云平台为基础架构,组建站点式健康信息采集设备、移动式信息采集设备和智能穿戴设备为实施核心的整个健康管理信息网,平台将以健康信息采集为始点,全面的对服务人群进行健康信息记录、健康知识教育、健康风险评估、健康方案指导、健康生活干预、干预效果评价、二次信息采集……的一个闭环式的健康管理解决方案。面向服务人群建立“自助、互助、医助”三位一体化的健康管理信息系统,是彻底地从传统的健康体检模式向检后健康管理的转变,从过去的临床疾病治疗为主,向疾病预防为主的转变。医疗机构需要做检后管理,以达到健康管理的目的。

作者简介

全科医生,主任医师,教授。从事临床工作28年,全科医学和社区卫生工作18年,现任深圳市宝安人民医院社管中心副主任兼全科医学科主任,深圳市全科医学培训中心副主任。主持省市区临床科研6项,核心期刊撰写发表10余篇学术论文,专著主编副主编3本,是深圳市全科医师专业技术能力评价指南、深圳市高级家庭医生胜任力评估指南、社康标准化建设全科医学行业标准、全科医学培社区基地建设与考核标准的主要组织者和撰稿人之一。2012年获得深圳市十佳社区健康服务工作者,2014年获得中国社区卫生协会组织评选的全国百佳“群众满意的社区医生”荣誉。2017年获深圳市”卫生健康系统十大杰出贡献奖。
