病历是对患者疾病发生、发展、诊断、治疗、护理、转归等情况的客观和系统的记录,反映医疗机构医疗行为的全过程。翔实、完整的病历不仅可以为医疗、教学和科研提供丰富的临床资料,也是衡量医疗质量、医院管理能力的重要内容,同时也是医疗事故鉴定、医疗保险赔付的重要法定依据。病历书写是各级临床医务人员的基本技能,也是医疗工作中的重要环节。统一病历格式,规范书写要求,提高书写质量,目的在于使病历更真实地记录医疗过程,更全面地反映医疗质量,是加强医疗质量控制的重要手段,也是医院现代化管理的重要内涵之一。原江苏省卫生厅于1981年组织编写了《病历书写规范》,并分别于1987年、1996年和2003年根据原卫生部的相关规定进行了三次修订。《规范》的实施,对全省各级各类医院病历书写和管理工作起到了积极的促进作用,为开展医院分级管理和医院评审评价等工作奠定了基础,为加强医疗质量管理,提高临床医疗、教学和科研工作水平创造了条件。
