作者简介

      高良武久

      1899年出生于鹿尔岛县。1924年毕业于九州大学医学部,曾任该校精神神经科讲师。之后于1929年转任东京慈惠会医科大学,1937年任该校教授。1964年任该校名誉教授。1940年开设高良兴生院。担任神经症的治疗。1996年5月20日逝世。高良兴生院于1995年关闭,阿部亨院长又开设了森田疗法诊疗所。

本书是我开始对神经症实施“森田疗法”以来积累的40年临床经验,并结合接受此疗法后恢复健康站立起来的人们的宝贵体验而完成的结晶。许多罹患神经症状而苦恼的人,接受森田疗法痊愈后写给我这样的感想:“现在反思起来我患神经症这件事对自己却是幸福的根源,为什么呢?因为通过治疗带给自己一个塑造人格的机会,使自己不仅从症状中解放出来,而且与发病前相比,自身对生活的态度大为积极,处理问题的能力大有改观,并且拥有了享受生活愉悦的那种从容感。”这确实是摆脱了神经症梦魇,洒脱挺立起来的人的写照。所谓神经症,是其人本身内在所具有的矛盾或纠葛作为不适症状在身心上的反映,因而在适应外界状况方面变得非常困难。但消除了症状,就不仅是回到发病前的状态,而且还自然地带来了新的发展,获得了比发病前更强的适应性。所以有已治愈的人说:得病反对自己是件好事。这话并非言过其实。恰恰是对症治疗所带来的结果。这里我说的“对症治疗”,就是我所施行的森田疗法。

体臭恐惧症
所谓体臭恐惧症,就是因为自己身体的某部分发出的恶臭,就不出现在嫌弃这种臭味的人面前。其实,体臭是主观的虚构性的,其本人的身体并没有散发出特别恶臭,完全是该人错误的判断。但是,无论作了多少次的说明,该人还是不理解,可见完全陷入了妄想之中。
患者深信不疑认为发生恶臭的地方,也是因人而异。有认为是自己的腋下,也有认为是出自性器官,也有深信是出自肛门的,也有肯定说是出自皮肤的。然而正如前面所述,不论检查多少次都无发现有什么异常。有趣的是,自己腋下确实是臭的人,本人是不会有什么感觉的,也不会有体臭恐惧。我曾治疗过的一名学生,深信自己腋下有臭,所以每天都要更换衬衫,实际根本没有什么臭味。认为自己性器官发出臭气的人,会到处去找泌尿科或妇产科医生。认为自己肛门有臭气的人也会以为自己肠胃有问题,会到处去找内科医生。
他们在乘电车时,也认为自己有臭气会让人讨厌。只要看到别人揉鼻子,或朝向自己腋下的反方向扭过去时,马上就认为这是在嫌弃自己的恶臭。家里的人尽管也不会感到有什么恶臭,但是当事人会认为“与自己有关”,不会与家人外出,令家人难以理解。体臭恐惧症的本人大多数愿意接受精神疗法的治疗。相信有恶臭也有深信其会散发恶臭的病人,如不愿接受精神治疗的,要完全治愈是非常困难的,在这种情况下,只能进行药物疗法。

强迫行为与强迫禁止
在此必须要指出的是,森田所谓的“强迫行为”指的是:意志薄弱的一种行为,是一种较难治疗的一种行为。
强迫行为中如不洁恐惧症患者,每日里会几十次仔细地洗手,这种行为是与强迫观念相随,是为了消解心中不安所做的行为。又如,凶吉恐惧者,会很多次地重复一个做法。强迫行为或多或少地都伴随有强迫观念。正因为如此,就会表现出意志薄弱的一面。事实上,有强迫行为的人正是森田疗法可以治疗的对象。
森田所称的“强迫行为”,是一种频繁出现的执拗行为,呈现出一种冲动的,并没有因果关系的行为。这类人想治疗的意愿是微弱的,因此也不太愿意听从于治疗的安排。普通意义上的强迫行为与森田所说的“强迫行为”在意义上是不同的。森田指的是治疗特别困难的强迫神经症。当然这两者在本质下的区别是没有的,仅是程度不同而已。
