    (一)传染源

    目前,国内外公认的布病传染源主要是牛、羊、猪等家畜。不同国家和地区由于流行株不同,各传染源的地位也不同。在中国,传染源重要的是绵羊、山羊,其次是牛,再次是猪;鹿的布鲁氏菌病不仅引起鹿发病、母鹿流产,而且可导致人间感染发病;另外骆驼、马、骡、犬等也可成为人间布鲁氏菌病的传染源。在广东、广西等省区,主要的传染源是猪。广东省冬季食羊肉的习惯兴起多年,从北方每年贩运大量的活羊到广东,近年随着牛羊肉价格的上涨,以及发展草食动物政策的推动,从北方引进种牛羊以及架子牛的育肥数量激增,导致广东羊种布鲁氏菌病发病增加。

    (二)传播途径

    人布鲁氏菌可经皮肤黏膜直接或间接接触病畜的污染物而感染;或食入生鲜奶、奶酪通过消化道感染,或通过吸入被污染的气溶胶经呼吸道感染。人感染途径与职业、生活方式和饮食习惯等有关。布鲁氏菌污染的各种物品及食品均可成为传播因子:病畜流产物如胎盘、胎膜、羊水含大量布鲁氏菌;病畜的乳、肉、内脏;皮毛,布鲁氏菌可在皮毛中生存4个月,传染期较长;水,病畜分泌物、排泄物污染的水源;病畜排泄物污染的土壤、尘埃。

    新中国成立前,布病在广东未见资料记载。新中国成立后,1955年发现家畜和人布病发生。广东省1956年把布病列为法定传染病管理,但直到1965年才有正式疫情报告。广东省卫生防疫站在广东发现的首例患者林某,男,28岁,为航海员(任会计),原籍福州市。于1965年2月10日因发热11天,入中山医一院,发热均在下午或晚间,约38℃~39℃,伴有头痛,热退后出汗,疲倦乏力。入院后3个多月来热度仍弛张于37℃~41℃之间。经用磺胺、链霉素、金霉素及输血等药物治疗均无效,热度越来越高。白细胞在2000~4000之间,血及脊髓涂片疟原虫阴性,布鲁氏菌凝集反应1/1280(+++)。患者于十年前有疟疾史,八年前患过痢疾。曾到过青岛、塘沽、上海、南京一带,因在船上工作,与运载山羊、猪等家畜有接触。
