  笔记1:外感风热的治疗要点

  外感风热之初,治疗在于清透,用桑叶等辛凉之味清透外邪,使邪从外祛;如见热势较重,可更加连翘、金银花等清轻而透之药,切勿一见有热就重用三黄、板蓝根等苦寒折火。过于寒,反使邪气闭结不能外透,从而变症自发、病情迁徙不愈。现在很多感冒,一治半个月不愈,很多情况就是用药过寒造成的。

  治疗感冒不分寒热,套以西医学"上呼吸道感染"一说,一路苦寒直折火势,已成为当前中医治疗外感的流行治法。要知外感之邪,当使邪从外祛才是正道,过于寒凉则肺失宣而不能祛邪外出。苦寒败胃,胃气受损无力运化而气血不足以祛邪,特别针对一些素体虚弱之人,更要时时审元气之强弱,慎用苦寒,风热外感慎用,风寒更是禁用。

  吴南京分析:

  中医学的精神是辨证论治,同样的外感风热,不同的体质表现会完全不同。如素体本有郁热之人,感受风热,风热之邪会和内在的郁热合邪,化热会很快,治疗之初再不仅仅是辛凉轻清之药以疏散了事,而一开始就得以辛凉轻清之中辅以黄芩之苦寒之药以折火,否则变症快速,津液一耗,气亦随之而耗(有人会说,现在有输液,不至于和以前那样的耗津。输液是可救津,但大火食气,风热和内在的郁结之热合邪必会耗损元气,所以热之耗津,所耗损的不仅仅是津,是气阴并伤。输液可以救津,但不能补气,所以不能把希望寄托在输液上。很多人一个小小的感冒,造成长久的免疫力低下,且时常反复外感,很大一个原因就是外感之时耗伤了气)。

  江南多湿多热,风热之邪,多在春秋之间,此时多热多湿,雨水也多,体内湿阻之人很多。湿和热合则生湿热,体内有湿热,再受风热之邪,在江南是很多见的。所表现的症状为困乏无力、身体困重不想动、头晕、心烦闷、大便黏腻、尿黄等湿热症状,又有稍恶风(或恶寒)、体温偏高等外感风热症状。此种风热常易当湿温为治,有人用三仁汤等剂治疗不效,用桑菊饮等治疗亦不效。治疗当化湿热和清透并用,利湿可以通阳,更加辛凉之味以清透之,效果理想。

  当女人月经期间感受风热,易引起胞宫瘀阻,而造成妇科病的发生。因为排月经是一个除旧过程,一受外邪肺气不利,肺朝百脉,肺气不利则血行亦受阻,于是很易形成瘀阻。治疗时要辛凉清透之中更辅以益母草诸调经通血药为治。

  治疗外感风热,一定要使邪外出,体内有瘀湿等情况,一定要进行分消,不能让风热之邪和内邪相合,这是治疗之大要。
