前  言
随着我国经济发展和人民生活水平的迅速提升,人群疾病谱发生了很大变化。过去常见的一些儿科血液系统疾病的发病率不断降低,而白血病及其他恶性肿瘤对儿童健康造成的威胁则日益显著,恶性肿瘤已成为儿童期死亡的主要原因之一。
面对这些变化,我国儿科血液病工作者及时调整专业重心,将恶性血液病的诊断、治疗放在了突出的位置,不断吸收、消化并改进国外先进诊疗技术与理念,始终如一地强调科学性、严谨性,开展了许多新的工作,积累了宝贵的经验,在儿科血液病的规范化诊疗方面取得了长足的进步。例如,开展规范的MICM 分型,特别是分子生物学分型;准确检测早期治疗反应,优化危险度评估,从而实施精准的分层治疗;不断将手术、化疗与放疗等基本治疗手段优化组合,广泛应用造血干细胞移植、免疫治疗、细胞治疗、分子靶向治疗等新技术、新方法,极大地减少了疾病的复发;支持治疗的熟练应用也使化疗的安全性得到很大提高;在不断提高患儿的无病生存率的同时,更加关注并提升长期生存患儿的生活质量。通过这些努力,儿童恶性血液病的疗效得到极大提高,急性淋巴细胞白血病和淋巴瘤的长期无病生存率已达到80% 以上,实体瘤也提高到60%~70%。
