前  言
初衷:大约医学院念到四、五年级的时候,我开始进入彷徨期。倒不是发愁将来进入哪个临床科室,而是害怕自己不会看病。内、外、妇、儿,一节节课上下来,“病因,病理,发病机制,临床表现,诊断与鉴别诊断,治疗原则”,貌似知道了不少,可是在见习中碰到了患者,总难把课本的一二三四,跟眼前的病例联系起来。这一彷徨的消解,自然与临床实习开始大量接触患者有关,但是在北大医院图书馆无意间借阅的一本书,也功不可没。这本书是一个临床疑难案例集,其中有一个案例,距今已有三十余年我仍然记得梗概:一个年轻女性,新婚不久怀孕,在产检时发现双肺粟粒状阴影,弥漫于肺野,同时有肝脾大,诊断为血行播散性肺结核,建议患者立即做人工流产,开始抗结核治疗。患者跟家属商量后,拒绝流产和抗结核治疗,但同意定期复查。当所有的医生都认为患者会每况愈下,终不得不接受流产和入院抗结核治疗的时候,奇迹发生了。定期产检的患者,身体状况一次比一次好,低热消失,体重增加,肺部情况好转。至预产期顺产一健康男婴,而产妇的肺部状况则完全恢复了正常。这样的“反转”案例当然是少数,但是按照教科书来生病的患者其实也不多。当时我作为一个医学生,就曾经萌发过这样的念头:如果医学教科书,都以临床病例方式呈现,会不会效果更好?
