前  言
我国已进入老龄化社会,国家统计局2020年2月28日公布的《中华人民共和国2019年国民经济和社会发展统计公报》数据显示,我国65岁及以上人口约1.76亿,占比为12.6%。按照国际惯例,65岁及以上的人群定义为老年人,而年龄≥65岁的糖尿病患者定义为老年糖尿病患者,包括65岁以前和65岁及以后诊断糖尿病的老年人。随着我国老龄化的加剧以及日益增长的老年糖尿病管理需求,老年糖尿病患者的管理亟须规范,但目前国内尚无一部关于老年糖尿病管理的权威指南。国务院办公厅印发的《中国防治慢性病中长期规划(2017—2025年)》强调,至2025年,糖尿病患者管理人数将达到4 000万人,糖尿病患者规范管理率须达到70%。
尽管糖尿病诊疗的一般性原则可能适用于老年患者,但老年糖尿病患者具有并发症和/或伴发病多、症状不典型、低血糖风险高、患者自我管理能力差等特点,在血糖管理手段和目标制定、药物选择原则等方面有其特殊性。国家老年医学中心、中华医学会老年医学分会、中国老年保健协会糖尿病专业委员会组织国内糖尿病、老年病及相关领域的知名专家共同撰写了《中国老年糖尿病诊疗指南(2021年版)》(简称《指南》)。《指南》依据目前国内外老年糖尿病领域的研究成果,同时参考相关专业学会的指南和/或共识,结合我国具体临床实践,经过多轮线下、线上讨论,反复凝练而成。自发布以来,《指南》在学界引起了广泛关注,对于指导和帮助临床医师对老年糖尿病患者进行全方位、全周期的规范化综合管理,改善中国老年糖尿病患者的临床结局具有重要意义。
