医药卫生体制改革必须立足国情,一切从实际出发。坚持以人为本,把维护人民健康权益放在位。坚持立足国情,建立中国特色医药卫生体制。坚持公平与效率统一,政府主导与发挥市场机制作用相结合。坚持统筹兼顾,把解决当前突出问题与完善制度体系结合起来。深化医药卫生体制改革的总体目标是:建立健全覆盖城乡居民的基本医疗卫生制度,为群众提供安全、有效、方便、价廉的医疗卫生服务。
2009年7月国务院印发医药卫生体制五项重点改革2009年工作安排的通知,在“推进公立医院改革试点”工作部署中明确要求“推行常见病临床路径”,随后,卫生部组建了临床路径技术审核专家委员会制定了22个专业112个病种临床路径,下发了《临床路径管理试点工作方案》。
“推行常见病临床路径管理”是“推进公立医院改革试点”的主要目标之一。是推进“医疗、医保、医药”三医联动改革的重要突破口,是兼顾医疗质量管理和效率管理的现代医疗管理重要手段,是我国医院管理的一次新浪潮。国际经验和国内实践证明,实施临床路径是改善医疗服务管理的有效措施。

第二章 组织再造与作业流程重组

一、组织再造
1993年,美国著名管理学教授Michael Hammer和咨询专家James Champy 合著出版了《再造组织:管理革命的宣言书》(Reengineering the Corporation:A Manifesto for Business Revolution)一书。该书出版后迅速在西方国家掀起了“组织再造”的旋风。有关专家认为“组织再造”是管理学研究的进展,是对泰罗“科学管理”的重大突破,将带动一场新的管理革命。
哈默曾经说过:“再造,不是自动化,而是重新开始!”其具有革命性突破的发现是:“科学管理”对顾客不一定是适宜的!
所谓的组织再造,就是指以顾客为导向,以作业流程重组为核心,对组织的整个营运过程进行根本性的再思考和彻底的改造,从而创造出一个工作的新世界(A new world of work)。
组织再造的核心:作业流程重组(Business Process Re-engineering)。
根本性变革(Starting Over):突破传统观念和习惯做法的束缚,以顾客为导向,重新确定做什么和如何去做。
顾客导向:令顾客满意。站在顾客的立场,评估作业流程的每个环节是否真正为顾客创造了价值及是否可以创造更大价值。
作业流程重组:作业流程重组是“一种有关作业改善的哲学。它是在对一个组织原有作业流程深刻理解和科学分析的基础上,以顾客为导向,对原有作业流程进行系统性重新整合或重新设计,系统增加作业流程中有价值的内容(Value-adding activities),减少无价值的内容(Non-Value-adding activities),以达到改善作业质量,提高作业效率,降低作业成本的目的”。
无论组织绩效改进计划的起始状态和性质如何,“流程”对于组织将注意力集中在顾客身上都是非常重要的概念。相关流程的重组能够极大地增加提供给顾客的价值,并有效地提高组织的整体效果和效率。(Peter F. Druker)
